Regarding the slipping back into habits, I've been trying to consider what triggers this in me; it might be a busy/stressful day at work that makes me want to just "relax in front of youtube", or a less than satisfactory face-to-face conversation, so I seek out deeper conversation online. With this awareness I look to implement tactics for improvement.
