I didn't play long enough to get to harder dungeons, but the I'd suggest nerfing defensive actions to give them a chance of failure for not using the correct action. Like: obstacles can't actively hurt you, but maybe Dex or Heal or Defend has a chance of doing some damage or lesser damage.
